Today… just for fun of course… I thought I would post a reminder of what should be nicely tucked away in your folder here in the Georgia office. As of November 1, these things are due. Which means… no worries… you still have 10 more hours!! 🙂 I know I know… don’t get all overwhelmed on me. It’s okay. A lot of you have sent me emails and explained where some things are and when I can expect them. I have a note of that… but please go through this list again and make sure you know where everything is!!!!
_ Your $150 deposit (this will be applied to the cost of your trip)
_ Your Commitment Form
_ Bio posted on Blog
_ GOOD digital photo (where we can easily see your face) submitted to AIM and posted on blog
_ Your Statement of Faith
_ Your Team Covenant
_ Your Profile Assessment Release
_ A copy of your support letter submitted to [email protected] – NOT OPTIONAL!
_ Deputized Support Raising Policies form
_ Myers-Briggs Test completed online and sent to AIM
_ A copy of your passport
_ Liability Release Form
_ Send out approved support letter to family and friends – meaning we have to have read it and approved it and sent it back to you with your return address labels and support cards.
_ Personality Assessment and LFYS Assessment
_ A copy of your Shot Records/Vaccinations
_ A list of Prayer Partners [at least 15 people] should be e-mailed to AIM
_ Submit the name of your Accountability Partner to AIM
_ Your Medical Release Form (sign the form in the presence of a notary public)
_ Emergency Contact Form
_ Living Will Declaration
_ Power of Attorney – Main
_ Power of Attorney – Finances
_ Proof of insurance must be sent in
_ $8,500 support due – At least $4,500 actual cash in your AIM account, Up to $4,000 may be in pledges turned into AIM – I will post a support update tomorrow on finances!!!
