Hey folks, these are some things that are coming directly from your mobilizer, Kim.  She wrote this post for me to pass along to you.
 Hey Squad!
 
We’ve talked before about a couple things that I’m going to need from you before we officially meet at your Training Camp. 
I’m still waiting on most of you to get me this information.  Your
Training Camp starts on July 2.  I need you to get all of this
information to me BY JUNE 10TH. 
 
IF I DO NOT RECEIVE THIS INFORMATION BY THE JUNE 10TH DEADLINE
YOU MAY NOT BE ABLE TO ATTEND TRAINING CAMP OR LAUNCH ON TIME WITH THE
REST OF YOUR SQUAD. 

If you believe you will have trouble meeting the June 10th
deadline on any of the following, please call me as soon as possible.

The items I’m waiting on are as follows:
 
 
A copy of your passport:
WHERE — Admissions profile  >  “My Mobilization Info” Tab  >  “Documents” Section
WHAT — A scanned copy of your passport and specific information from your passport.
CHECKLIST — (If either of these are not fulfilled, you may need
to send your passport in to the State Department.  If so, call me
ASAP.)
  • Minimum of 13 empty pages in your passport.
  • Expiration date AFTER January 2013.
 Information about travel to Camp:
WHERE — Admissions profile  >  “My Mobilization Info” Tab  >  “Travel” Section
WHAT — How are you getting to and from Camp as specified in the Training Camp details?
CHECKLIST — If you have already uploaded the information, check
with the airline before the June 10th deadline to be sure they have not
altered your flight information.
 
 
Myers-Briggs Personality Test:
WHERE — Admissions profile  >  “My Mobilization Info” Tab  >  “Personality Tests” Section
WHAT — Complete the Myers-Briggs personality test online.
CHECKLIST — One you have completed the test, make sure the
results have uploaded.  [Don’t worry about the “Leading from Your
Strengths” test, you don’t need to complete that one.
 
 
Profile Photo:
WHERE — Admissions profile  >  “My Profile” Tab  >  “Edit Profile” Section
WHAT — A clear photo of your face.
CHECKLIST — 
  • Photo must be clear enough that we can make our your face.
  • It’s best if the photo will print well in black and white.
 Medical/Counselor Release:
WHERE — Admissions profile  >  “My Mobilization Info” Tab  >  “Medicals” Section
WHAT — A release from your doctor or counselor as has been previously discussed.
CHECKLIST — If you haven’t discussed this with someone
previously, you shouldn’t need to worry about it.  If you’re not sure,
call me.
 
 
Proof of International Health Insurance:
WHERE — Admissions profile  >  “My Mobilization Info” Tab  >  “Documents” Section
WHAT — A copy of the card you receive from your International Health Insurance provider.
CHECKLIST — There is a link to a plan that meets World Race
requirements at: Admissions profile  >  “My Mobilization Info” Tab 
>  “Preparation” Section  >  “Insurance” dropdown
